It is a little of both. The unit of measure used also dictates the map size in the engine. Yes, they can make the map bigger by changing that measurement. The drawback is that it is a major overhaul of the game. It will take time and money and has been discussed in the past. Since it will really mess things up in the game, it has been ruled out.

The underwater city is there as well. When I was playtesting on the TestLive server, I was able to look into the desert at the edge of the map. This was fixed prior to the actual release of that dungeon.
The Sewer dungeon is in the Southwest desert where the place where you are on the cross is located during player creation. It is used for the cut scene when Conan cuts you down from the cross.

Uh, there isn't "just one" up there. Literally can just load up the dev kit and look. If you are going to call people out for misinformation, at least don't immediately start doing it yourself.
But yes, there is a tiny bit of grid space that can be "used". What Narelle said is correct. It comes down to importance, assigning people to add that tiny bit of grid space instead of working on other things (which you all will immediately scream about how they wasted resources, because that's what you do), time allocation, resource management, performance concerns on 8 + year old consoles and people who run on minimum requirements, etc.
Why do gamers* always believe things are simple?
There can be numerous reasons why that area is unused in the NE area but the reason of that's where all of the dungeons isn't one of them.
Edit: I respectfully withdraw my argument about one dungeon being up there, there are 3 dungeon interiors, Volcano, Warmakers, Wine cellar.
