Rescue it.
I have occasionally "lost" a thrall shortly after getting myself butchered by enemies, only to get back to where my thrall is (as shown on the map) to discover the thrall has mysteriously gone invisible. (Trust me, while in SP mode I've gone into ghost mode and looked everywhere around the spot where my thrall should be and did not see it.)
When that happens (and it hasn't been often), I return to my base where at that point I can rescue it at the cost of any items I've placed in it to carry.
Believe me that can sting a bit especially when the thrall is carrying a couple thousand iron ore or something else I particularly value.
But, if you have any attachment to your thrall, you'll want to rescue it in spite of the occasional loss of materials.
If you don't know how to rescue a thrall yet, do this.
Press "i"
Select the Followers Tab.
Locate your thrall (or pet) in the list.
Right click on the line and select "Rescue".
If it hasn't been attacked, it will show up near you minus anything that was in its inventory.
If it has been attacked, you'll have to wait for a timer to run out before trying again.
If it is just gone you can try the Rescue command. If it is not in the followers tab it was likely killed, check the event logs with the range set to 10000 to find out.
I'm playing in single player mode. In my opinion this occurred since implementation of "Isle of Siptah".
