You said the thrall is holding the truncheon in the off-hand? That's really odd, I don't know if I've heard of that before. If that's the case, then I'm guessing whatever's in the main hand (unarmed, probably) is what's actually dealing damage.
If a Thrall is putting a Truncheon into the offhand, then its because its an offhand (and probably modded) Truncheon meant for the offhand. The system doesn't allow for free form swapping of weapon hands with the same weapon. And as Dread Swoop said, its using the main hand (whatevers there, even if its nothing) to determine the damage, not whatever is in the offhand.
Give them a proper truncheon for them to use in their main hand, and that will solve it.
also the off-hand is strange so that may be a moded thing and could be an issue with the mod as to why it kills instead
or solution: let the npc reset its hp by breaking agro for a moment, since KO progress does not reset, this is easier to do solo than with a thrall however
