As for single-player/multi-player, yeah, ALWAYS place things/gear into chests/boxes/vaults and be sure to lock them. Perhaps things would be more pleasant were such 'precautions' not the 'go to' prevention method when it comes to losing items/gear but it is what it is. I've lost multiple things by logging out and or being away from the computer long enough to die and lose said 'multiple things'. I chat with people both 'in-game' and 'out-of-game' about lost items and what is described above seems to encapsulate the problem and potential 'solution' in one paragraph.
The reason(s) for losing items despite being 'safe' are numerous enough, here are some: Temperatures, hacking/exploits, dehydration/starvation. As someone who has been attacked through walls 'I' am aware of the possibility of dying where I 'shouldn't' and as such I 'try' and, at times, fail, to remember to lock things up before I log out, sometimes even if I'm still planning on 'playing' because things just seem 'iffy' on the server.
Too Long, Didn't Read Version: Lock up all gear and items in boxes/chests/vaults before logging out.
For what was 'lost' I am sorry.
When i started the session, my character was not where i left, instead i spawned at my bed with nothing on me and haf of my life, as if i died.
The curious thing was that the thralls in my wheel of pain had several hours of progress more as when i last left singleplayer.
Can it be, that the singleplayer session wasn't closed properly and continued in the background for as long as my pc was running? that would perfectly fit the situation.
