For knives, stay away from the Eternal Reward (you really need that first-strike disguise). I think the standard is the Big Earner, to ensure you get your DR back in time.
Revolvers, these days, l'Etranger seems good, since it gives you extra cloak. As far as I'm aware, Spy pistols don't have all that much use in MvM.
I don't actually play Spy, this is everything I've picked up from dabbling with him briefly and from reading everything people say on here.

kunai is really really good. Just make sure to not get picked between crossfire.
you will also need to understand that alot of people are not tolerant. you will need to prove your worth, and to do so you will also need to understand how the AI behaves vs human spy
Kunai is worthless, just get health on kill. If gun doesn't matter atleast use etranger for a passive boost.
knife. use stock or big earner (earner only once you understand what not to do) (as in don't use it until you are decent with spy. 25 health is alot)
sapper: red tape. for mecha or else it doesn't matter. (hope they haven't fixed this yet.) (will test when i play spy.)
or you could use the kunai, guarantee +150 hp per kill and save 800$
etranger doesnt matter since you dont need to be invisible. You only need the DR's damage shield, which isnt affected by the letranger.
Assuming you've already encountered Uber Medics, you can just disguise, jump right in, sap, stab, cloak and get out. It's recommended you upgrade the sapper to increase sap range (There might be four latching onto one giant) and duration. (Unless you have fast attack speed, you'll need about a couple of seconds time to finish off the Uber Medics if handling more than 1) Spy vs Engie tactics also work well on Bigrock maps, so don't be afraid going spy there if neccesary. :)
But I can't help you on what set is best.

Health regeneration is good for healing through chip damage, but doesn't really do anything to keep one or two solid hits from killing you, not without damage resistances anyway. The other knives with overheal at least give you a small health buffer to keep you from being one-shotted. And the Kunai does what it does for free.
Even if I believed that, what would be the justification for using the Big Earner? The Kunai's overheal comes into play every bit as often as the Big Earner's cloak regain.
health regen isn't healing though chip damage. think about 6 health per second and your health pool of 100. that's 6% of your health per second. add that with resistance and dead ringer. you will rarely die.
big earner justification: there is one use which I find very useful. when the tea gets pushed back and there are uber medic + whatever targets. you really only get 1 or 2 stabs in before DR away. when you uncloak and get amo you are not at max cloak. if you wait that extra second the robots will start to advance and slowly move farther away from the amo. with earner you can maintain you location while killing the medic/heal target. if it's smaller robot pair (ex: heavy medic) you can easily sapped a pair and get your cloak back. it makes it much more convenient.
when i play i usually play with no scout so i don't have the time to deal with smaller robots. I get the money and kill medics/engies if it come to that (giants are next). if we get pushed back the team knows (pyro usually) to get the money near spawn.
maybe it's just a playstyle difference but I play more get money/kill key targets. you may play more aggressive which would be more justified toward using the kunai.
My favourite loadout is kunai and enforcer but the loadout doesn't really matter, just use whatever you think works best for you. :)
