The one in custom is useless.
Also check if any of the folders in custom contain a cfg folder with a autoexec.cfg inside it, as those will override yours.
If you're using mastercomfig, your personal autoexec needs to go here:
tf/cfg/overrides/autoexec.cfg
If you add your personal stuff to that one and then update your mastercomfig, it'll get overwritten and your stuff will be lost.
