Worse than the default Shotgun. You rarely, if ever, need all 6 or all 8 shots of the Shotguns whenever you pull it out. Normally your target is dead after 2-3 shots, since you've pulled your Shotgun out after a quick engagement with your primary weapon.
The default Shotgun does more burst damage and that is generally what you need in standard gameplay. By the time you get off those additional 2 shots, either you or the target is usually dead.

but sometimes a shotgun is needed and always using a sandvichis kinda boring( and useless if you are being pocket by a medic)
Sanvich is incredibly useful if you are being pocketed by a medic. You can save your medic's life with it by throwing it down for him. If you think it's useless you're playing heavy really wrong.

Roll with a Sandvich and keep your medic fed. He will appreciate it.

tiny bit better in one department, tiny bit worse in another.
Boring wep, though balanced.
Both can 2-shot 125hp classes but Shotgun can it more reliably.
In prolonged battle Family Business does actually more damage with full clip than the Shotgun.
I´m not sure why but at mid-range for me it feels like Family Business is better could be placebo effect or something.
One thing I´d like to see for Family Business is slightly tighter spread so it is slighty better at mid-range while Shotgun is better at close range, though both are are still useless for serious Heavy gameplay because Sandvich exist...
