At present there is no way to get additional slots; although you could use scripting to automate equipping stuff IIRC. The latter is a messy way of doing it though and would require manually editing and binding keys to the commands each time you want a new one.
Ideally I'd like to see some "locked" load-outs for each item-set, which you can "unlock" by slotting all the set-appropriate items into them. This would free up A-D for customs.
There is only 1 way of getting more than 4 slots, which requires scripting. For some reason,you can use console commands to access a 5th loadout. It's a bug that basically can only be accessed via commands.
I don't agree on the locked loadouts part, as there would be a problem with the market/ economy. There would be hardly a reason to trade in tf2 if you kept your set forever.
What I meant was - in addition to slots A>D - have a slot for item sets like "Airborne Armaments" & "Saharan Spy" that require the full set to unlock.
This would reduce the burden on slots, it would promote item-set use, and it would help the economy by increasing demand for many uniques - and indeed item-set based stranges too.
