Also make sure you have that your current mic is set to be the one being used at the moment in the steam settings. And in the Team fortress 2 game options that is under the voice tab.
Maybe it seems stupid, but if you use an headset, did you connected it properly?
I was in a hurry earlyer writeing it but here is a redone way of doing it that is step by step
Step #1. While on Steam itself click on the far top left on the word that says steam it should be above the back space arrow that is to the left of the word that says store.
Step #2. Click the word settings. It should bring up a menu that has a list of different things. Such as Account, Friends, Family, Ingame and so on.
Step #3. Click on the menu that says voice.
Step #4. Click on detect audio devices.
Step #4 alt. Click Change device to do it yourself. I never had to do this one my self but I am not sure if with some mics or web cams if you need to.
Step #5. Click on where it says test microphone. then speak into the headset, microphone, or webcam.
Step #6. If all went right you should hear your voice repeat back at you.
Step #7. Move around the thing that says microphone volume that should change how loud or how soft your voice comes in to others. Change to what feels ok to you.
Step #8. Move around the thing that says receive volume to what feels ok to you. Receive volume you will not really hear until you are in game if I remember right since that affects how much you hear others who speak.

this worked for me, maybe not for you...
