TF2 stutter with consistent frame rate on good pc

tried high & low settings from comfig (high runs better overall), tried v-sync etc.

4080
amd 7700x

I like to play with stretched 16:10 res on a 16:9 monitor. Switching back to the automatic resolution helps considerably.
How can I use a stretched resolution without the stuttering issue?
