He's called Heavy because he's the Heavy Weapons Guy. He specializes in Heavy Weapons. It's not because of his weight. It's the same with calling the Demolition Man just Demo or Demoman.

He's also a video game character, no one cares even if people get offended by what you call him. More so when he himself doesn't care about it. You don't see him crying when Scout tells him to eat a salad.
