Report a translation problem
So you go to steamapps/common/team fortress 2/tf/custom.
Then, if these files are not there, proceed to make them inside of custom.
userstuff > scripts > items > custom_texture_blend_layers
Once in the last folder., name your photo badge picture Photo_Badge_Overlay, save it in PNG format, make sure it's res is 128x128, DO NOT USE 256x256 or IT WILL NOT WORK!
Apply decal once that is done, it should be the PNG picture named Photo_Badge_Overlay.png
If this STILL does not work, go to TF2, properties, and verify the game cache.
If the color is off, go to the white and black color sliders when applying the decal and make sure that it is ALL the way white.
What size file do you need for a sign?
TF2 MUST be closed all the time when youre doing this.
Its very similar to the custom decal for the objector
There is your response to the size of the PNG image.
It applies to basically everything, and I mean Everything.

Yeah... The 64-bit update broke this method. Currently I have been unable to find any current method to do it that is this simple, but there are very complex ways to do it though.
