V-Sync is known to introduce input lag. You're better off capping FPS with fps_max <value>.

So many reason but if you want a fix, Just Set v-sync ON (double or triple buffer if available) if you see some input lag or something seems weird, just lock the game manually at 60FPS with fps_max 60, (if you have a high refresh rate monitor, lock the fps to the HZ of your monitor.) Also, If i remember correctly I think you have to make this every time you open TF2 ..
Edit: try the settings offline first.. If it's working offline but not online, it's another problem that has nothing to do with graphical settings

I had the same thing and I updated my hud and that worked
Try turning on raw input for mouse settings. Made all the difference.

The latest stutter I fixed was from a monitoring overlay i was using. One that shows fps, cpu/gpu usage and so on. It was Rival tuner that runs with mis afterburner. Took me a week to pin it down to that. Another one to try is -refresh 144 in your launch options or use whatever your monitors refresh rate is like -refresh 60 -refresh 175. The game Should recognize your refresh rate, but for whatever reason sometimes it does not. Finely, once i got an issue where my display port cable was failing intermediately causing it to only display at 60hertz. I replaced it with a dual link dvi cable cuz i only play at 1080p anyhow and it fixed it back to 144hertz that time. ie there are a bunch of things that can cause stuttering in games, but maybe it could be one of those things. good luck
