As far as other syringe guns... I have to put the Blutsauger first, and the Overdose next. Stock is not outright bad by any means, but the lack of added effects just pales in comparison to the negligible downsides of the other syringe guns.
The Overdose's speed boost only starts being effective when you have >70% uber, in wich case you might as well just try to use the ubersaw and pop it on a random teammate that happens to be there.
And 99% of the time you won't have that much Uber AND be alone, unless you decided to charge into the enemy team. And even in those suicide cases, even if you are a crazy-ass like me and don't like using the Ubersaw, wich would allow you to attempt an Uber escape and again turn the Overdose's upside irrelevant, you deserve to die anyway.
The Blutsauger is painted bad because "you only hit 25% of the shots at most, so the downside is much worse than the upside", but the secret is aiming at a slow/unaware enemy to survive a couple seconds more while backup arrives, instead of clumsily shooting at the Scout that is chasing you (if it's not a Scout, you can just outrun them, and they will be easier to shoot, especially pyro, that is forced to come at you on a straight line to damage you, and soldier and heavy, who are too slow to dodge the syringes), that you won't be able to escape from nor defeat in any realistic scenario.
And the Syringe gun gives neither the chance to run away nor the chance to hold your ground, so...
On the contrary, the Vaccinator shines the most with the Crossbow. The Vaccinator, much like the Amputator, was designed for a defensive playstyle. The Overdose becomes redundant as you can just pop one of your extremely disposable mini-ubers to escape if needed.
Plus, because of the reduced overheal, you will need to heal a lot more often, wich makes the Crossbow invaluable (Or you can be a hipster like me and attempt to play Dispenser Simulator 2018, but even then the Crossbow is nice to have on your side as a reliable source of healing).
EDIT: Just tested to be sure, the Vaccinator does not make you get +80% nor makes you get the top speed at 25%. Having the charges split does not make them count as 400% charge.
I'd recommend use the blutsauger, the downside for it is not working for some reason, even if you miss it would not drain your health.
You have to aim at ridiculously high angles and lead targets stupidly far ahead because of the slow speed equalling a shallow arc.
It's often why just pressing s+a+d while shooting your syringe gun isn't as effective as some say, not that it even should be in the first place.
But still, they're a bit too slow moving. The travel time is just far too high.
You can see the sryinge coming and dodge it because the projectile is that slow
