How does one collect souls?
Is it on every kill?

you get them from kills, but if you die you lose all of them and have to retrieve them from where you died.

You don't have to retrieve anything. As long as you don't get killed immediately after making a kill yourself, the soul will be added to your count. If you die after the soul has been registered, you don't lose anything.
