- Limited map pool, not everyone wants to play 5CP and KOTH. Those who want other modes are simply screwed. Everybody hates Turbine trying to be competitive
- Too high graphical restrictions. The enforced default setting is too high for some players, limiting their ability to play Competitive optimally
- Lack of class limits, making for stupid and unfun combos in a 6s format
- No reward system, why bother ranking up to Death Merchant if you get nothing for it
- Prevalence of cheaters and kiddies, the weak entry requirements let the lowest denominator enter Competitive too easily instead of filtering for experienced players
- Unoptimized gamemodes; CTF is flat out broken, Payload and 5CP are too slow and Attack/Defend has horrendous maps
- Domino effect of "If nobody's playing then I'm not playing." The cycle repeats over and over until virtually no one's playing it

People play MvM overwhelmingly because of Aussies. I guarantee you MvM would've died a long time ago if it weren't for Mann Up mode to sucker them in.
I don't think Overwatch's competitive scene is as healthy as people think it is. It's basically being sustained by Blizzard's checkbook.
It'd make Casual players have a stronger interest in Competitive. By hooking them in with the promise of rewards through the built-in system, some players would develop an interest to invest themselves into the Competitive mode and community.
This has been proven with MvM and Mann Up where a tiny minority of players still play it solely because of rewards.
But something like a hook, like a badge you get for playing 10 placement matches, or maybe exclusive random drops you can only get during competitive gameplay (that are not tied to palyer performance) might work.
It doesn't have to be a lot. Just something to get people to try it out.
Pretty much all of these issues are present in Overwatch too though.
