Every custom hitsound is either inaudible or earrape
I have had like 6 different hitsounds and most of them were completely inaudible, but when I downloaded the Quake Hitsound everyone uses, it was extremely high pitched no matter what I do. The Quake Hitsound being earrape

make sure the pitch is set right in advanced settings

sv_pure 1 servers will do that to sound mods. not much you can do unfortunately.
