How do I get the skull island topper.

I recently defeated Merasmus, however this did not give the skull island topper. People tell me that I have to get to the top of the skull on ghost fort, however I am still unable to get out of the fenced map area, and no one on the game was willing to tell me. How do u get the hat?

achievement, search for it on the halloween achievements

IIRC you get it by being the first to the bombinomicon. If that doesn't work you should get it by beating merasmus

I thought it was from one the reward portals? (like getting the monoculus from Monoculus)

Oh, u gotta go through the red bombanomicon portal, that explains it. I must have missed it, caus somehow I don't remember seeing it.
