I guess common idioms of the english language have taken a backseat to memes.
First off, it's literally a banana. That's the basis of the joke.
In the Meat VS Match update, the Pyro won, making the Heavy place in second. A person who is in right under someone else is a second banana. The Heavy was given a banana named the Second Banana.
Also, this is the second banana item released in the game, but that part is probably just coincidence.
You can just google "Second banana" and Google'll tell you that it's an idiom that means second place.
