do i have to fight the enemies in ghoul ruins till it drop, or chest?

I found it in a chest. (South West of level -1)
When you open the chest, it'll ask you if you want to bring it back to town.

Is it still possible to get the gems out of the chests that you have previous opened? I have done all the chests during missin to the ruins.

Even if the girl gave you hard to understand directions, there is no way that you could have opened this chest.

For anyone else who is wondering, according to someone I watched play this game:

You have to go down the stairs, and it's a few screens to the right of where you went down the second set of stairs. It wasn't there previously.
