Worlds that are blatantly catering to the lewd crowd rarely make it out of the community labs, or are kept private.
Avatars that are of the same kind tend to get removed relatively quickly if the ones wearing them are inappropriate with them in public instances.
That leaves the other players; most are sensible enough to not act inappropriately in public, especially around minors, but there are also some who just don't care.
Luckily, there are thousands of different worlds in VRChat, and you can make a private instance of any of them, so you'll most likely find somewhere you like, without being surrounded by the people you don't want to be bothered by.
The game its self is probably teen rated in public, mainly do to cursing and subjects talked about... but sometimes that takes a sharp dive into, well lets just call it 'internet culture'.
