Malady not at Black House (Spoilers for Doctor's Orders)
So, the first time around I went to talk with the Doctor after getting an invite. Malady wasn't there as she was supposed to be and I went and spoke with the Doctor. I provoked him by saying his name and then refusing his deal so he attacked me (demon form) along with his demon minions and nurses. I killed him, no problem (expoler mode, obviously)
Apparently I'm not supposed to fight him there? But I can't get Malady's thing t trigger. Even in the Hall of Echoes she just talks about finding Dallis. Any ideas where she might be in Arx and how to get the Crippling a Demon quest?
(Also, Jahan wasn't in the basement either. Just a dwarf prisoner) And I don't have Lohse in my party so I have no clue what to do to get back on track

I thought that whole quest was related to Lohse? If she's not in your party, maybe you shouldn't have gotten the specific quest log. Playing with Lohse, Malady only cared about the Doctor because of the danger to Lohse. Otherwise, he's just another bad guy.

Really? Considering guides and stuff I figured it would be the same for all characters. At least it's not a bug or sth

Don't overly take my word for it. I appreciate the amount of "choice" they add into this game. But some paths seem to be broken or at least inconsistant with tyhe prdicatablility / storyline they've achieved in the rest of the game.

Lohse is in my party and I have done all prerequisites for the quest (hardest mode below the disabled save) and no Malady, nor is Jahan in the dungeon.. I cannot get the Demon's armour off before he turns lohse, so pretty much stuck.

I didn't get Malady either, had to beat the doctor the hard way.

Malady's involvement with the Doctor is a mini side quest that is part of Lohse's origin story. If you don't have Lohse and haven't been working through her origin story, Malady won't appear here. No Lohse also affects Jahan's involvement. You can and probably will run into the Doctor without Lohse, but it plays out a little differently.

Spoiler alert: The Doctor is the demon posessing Lohse.

By the way, the Doctor can't turn Lohse against you if she's immune to charm. In my run, her headgear had that ability, but there's nothing stopping you from using an immunity to charm potion on Lohse before the fight begins. Just in case, carry an extra in case you need to use it more than once.

Also, no one said you have to start the fight inside the house. Teleport a nurse out and start the fight on the porch. The Doctor will get to you and fight outside, but you don't have to fight everyone at once.

Potion of Strong Will does not block Demonically Possessed placed on Lohse by the doctor.
