I really have to re-do my playthrough because I missed quite a few things and the whole soujar thing I didn't got it (I did the 3 necromancers mini dungeon and never found any souljar ...)
Exact same for me.

EDIT: No, she still dies. Maybe Amadia turned into a death goddess :D
For me, the statue as Fane blessed my character. If I tried it with any of my other 3 companions it made the water underneath turn into healing water, which is what transforms the pig back into her human form.
I gave the jar back to Gratiana as well and it was fine.
Talk to the statue and make sure you select the dialogue options to listen to it, feel it, weep with it etc. If you've already done this, you can do it again by getting other companions to talk to it for the first time. If done correctly, it will turn the water into healing water, healing the pig of its curse.

That is however a very poorly designed quest, like many of the quests in this game. There is zero reasons what so ever that I would ever need to use her over my primary PC, or even NPC Fane does not work. Nobody should assume to have to repeat talking to a statue 3 times with different characters for a quest to complete.
Worst yet, once you convert Feder to human state again... SHE NEVER EVEN THANKS YOU. Like come on.
Such a shame for a great game.
