help - game breaking issue

I am on the nameless isle and trying to get into the sallow man cave but I never got a cowl of true sight from alexander and I've lost his head as well

is there no way to continue the game now??

That's an optional side quest. It isn't going to stop you from progressing if you don't do it. If his body doesn't have it on him, then you probably can't proceed with that quest. Just ignore it and continue to try and enter the mountain. The Sallow Man isn't required.

I mean, what did you do with it? It doesn't despawn and there's no way to permanently get rid of something aside from literally throwing it somewhere you can't recover it. If you sold it, its still in the vendor's inventory you sold it to. If you left it behind, his body is still there to loot. Nothing goes away.

There are several different ways to get through that questline. Ditching (or eating?) the head of a major character that another character asks you for, somebody with critical importance to the plot, is a pretty reckless action on your part. Does the game really need to spell it out for you that the head of Alexander actually has use?

True, but typically anything you need the part for is obtained by eating it. Was also more referring to the cowl, not the head.

Either way, that doesn't change the ultimate endpoint here. Just move on.

erm, The Sallow Man literally tells you to bring Alexandar's head to him :X
