[SPOILERS] How to fix levers on the Blood Island?
Do i need a metal bar or something?

go to ancient forge and use the silver bar. it will create the missing parts for the lever.

So I created one of the levers, tried to combine it with one of the broken ones, and.... it just creates "?". Am I missing something?

Try a different order of the tomb or a different lever, or leave and come back. I was having the same issue, went to a different one, completed it, and then went back. Also, did you talk the ghost into approving your entry yet?

1 silver bar is just on the island on the surface somewhere

Another silver bar is in the middle delve in 1 of the crates or jars or how was it..

You can use silver bars or silver cups to create the levers, items sold that you may want back will be with the vendor you sold it to, so no worries there.
If you only get a question mark when putting the lever into the slot, make sure the person doing that actually has the lever in their inventory, as for some reason "magic pockets" doesn't apply to crafting levers (probably because the finished item is not in your inventory but in the environment)

If you still can't get in, convince the ghosts to help you, if that fails, consume the spirit, but you'll be forced into a fight on the other side if you force your way in.

Just ran into this myself, complete two of them but now i'm at the last one and the lever when i attempt to fix it shows this question mark instead of repairing the lever like it did before?!
