I've run through the main quest 5 times or so, and never had such an issue. Not sure what could be causing it if you have no mods.
Regardless, shouldn't be a big deal. You can always get Spirit Vision and Source Vampirism on the Nameless Isle if you miss it in Driftwood. You can get an achievement in Act 4 for ♥♥♥♥♥♥♥ up the Act 2 questline, so the game already assumes you may end up leaving Driftwood without like 99% of the stuff you came to Driftwood for. XP isn't really an issue either. There's plenty of XP in the game to compensate for missing a few quests in Driftwood.

There will be a destroyed academy on the nameless isle that you go inside of. There are some slabs with text on them and interacting with them teaches your character spirit vision. If you missed it in act 2, I would have to guess you also missed source vampirism, which also on one of the slabs in that room, right next to the spirit vision one.
It's an essential quest that everybody must do. None of us have any ideas of how its even possible that you missed it, that's not bad game design.
And it is bad game design if you can miss quests that are essential to completing large portions of the game. You basically have to figure everything out in this game, there is no hand holding, which I get, but they should have made it that these "essentail" skills cannot be skipped. And it is possible because I don't have them and I am basically done everything I can do without it in act 2.
Except no one but you is even sure that:
1) You actually missed the skill
2) You didn't do something weird like run mods or edited a savegame that may have influenced your game, leading to alleged miss.
I have never seen what you describe, so personally, I'm very hesitant to call it "bad design".
Of all of the forums, and even a google search does turn up some references to missing the spirit vision skill, but those are so few compared to the millions of runs - literally millions of them, that it's pretty unclear how this might have even occurred (assuming it really did).
So what you've experienced is exceptionally rare and there are likely pertinent details about how this may have happened missing in this forum thread. Those details are likely not related to game design at all.
Edit: I also notice that you have completely ignored the questions about "how did you exit act 2 without getting it?". This is pertinent info. See this video, timestamp around 19:15:

So did you skip the entire Meistr quest or what?
So maybe lay off the "bad design" accusations until you actually know what happened.
Anyways I know that I wasn't using mods or any other modifications as I have been getting achievements playing which you don't get with mods, feel free to check my profile. I still stand by my statement that it's bad game design that this can happen. And quite honestly I was about to stop playing because of it.
