Lohar isnt giving me the source hammer!?

Every playthrough Ive done Ive gotten this weapon, its apparently supposed to be scripted. But this time Im not getting it. I got some garbage axe 4 levels below mine, and a key that opens a chest with a pittance of loot.

Whats the deal with this, are there certain conditions that must be met to get the hammer? Really need this weapon for my 2 hander tank, its ridiculously good and Ive been waiting for the damn thing this entire playthrough.

Did you give him Mordus's Amulet? That is what always triggers the hammer for me.
