How to turn skin graft scroll into skillbook?
I have the scroll and a blank high tier polymorph skillbook and tried combining them but it didn't work

I'd recommend keeping the scroll and buying the book. The scroll is quite useful.

There should be a vendor, unless he died for some reason. Maybe the guy on the docks behind the toymaker house?

If you are taking about Trader Victor Voss, he had no polymorph skills.

Maybe, Most of my games don't reach Arx and I always have all my skills setup before act 3. I tend to go thievery crazy so I'll have every single skillbook 3 times over and some million or more gold to last the rest of the game. SO I really don't know the vendors. I'd guess maybe the elf near Saheila? Dunno.

The crafting recipes don't always work. Dunno why. Some recipes just don't work when you'd expect them to.
