Lohse snuffing out the candles....
Is this a bug or it that because of some previous decisions I made?
Lohse will snuff out the candles at the doctors place no matter what.
I only get two or three options when clicking one candle. I reload and no matter what I choose, she always snuffs the candle out.
Any workarround?

Have you snuffed the first candle? The very first candle will determine the rest of them if you said no to malady then she will abort the quest. If you have an earlier save reload go with malady to the realm again and decline to sniff the first candle. Hope this helps.

It's weird. I couldnt prevent Lohse to snuff out the first three candles no matter what. However in the last section with the 1000 candles I could say I will leave everything and we did. Leaving the last candles alive! I dont like those trial and error save/load sections ;)

It's a strange one for sure- I did a pretty decent 'good' run, but she snuffs them all no matter what too. The workaround I used was to avoid malady with Lhose separated from my party, TP her in once I am away from her, therefore never starting the candle section at all. I was REAL determined not to kill thousands of innocents on this run.

The full power adramalik fight is ridiculously hard, but if that's what you are wanting to do, I'd suggest using the 'Voidwoken fish exchange' scroll to get into his basement and facing him from the door to his left. It just works better position wise in my experience.

Eeeh... Not really that bad... There are cheese strats, but a decent team shouldn't have much trouble.
