It's been my experience that with just a medium amount of side questing and exploration all mobs and fights i've run into were the same level as my party. I did fight a voidwalker two levels above me when i was level 10 but i did realise at the time that it was probably a bit early for that fight. I still won lol.. .
So what i'm saying is that on tactician mode, and i'm level 11 atm, you probably want the enemy the same level as your party and if the mobs are too high, explore around a little and talk to all npc's 'cause there are a lot of side quests.
The game looks hard on the first playthrough but when you take time to have a good gear and a good build it becomes a LOT more bearable.
If you haven't gone up north to bloodmoon isle, look around up there and save often. There is some great quest xp there, but also a few high level fights you want to avoid. Also has a trader with level 15 str chest armor that you can buy at any level, and will help out a bit.
As to your last question, yes. Get gear as close to constantly maxed as possible. Weapons (aside from staff/wand) should get priority, and most armor in most parts of the game can be fine 1-2 levels under you *if* you get really good bonuses from it, but should still be as high as you can get it.
A minor spoiler about Wrecker's cave (that is to the west of driftwood, not north btw):
You're supposed to lose the first fight where the voidwoken web you, so don't worry if they apparently run all over you and keep going.
You missed at least 1 and half levels of xp in the first act.
Except for that troll but it was 4 on 1 so that don't count,lol.
Edit: To elaborate, there are times you might stumble upon a fight or a side quest that is 2 or 3 level above you (like the scarecrows in act 2),but there is never a time where you are forced into an encounter like that. You can always leave it be, level up via appropriate leveled quests, then come back at the right level.
