Do you have to permanently kill a companion in the arena?
To impress Nebora so she'll remove the collar, do you have to kill a companion in battle?

And can you fight another player instead of an npc for the title?

Only one can win. Also, not even a good idea to get it removed there unless you're ready to leave the fort.

Arena fights are not to the death. You can pvp with your friends in safety at the arena. You also get the option to challenge friends after your first run through.
