Always caught stealing.
So what's up with this? I have read many workarounds but everything I seem to read is from around when the game launched in September...

How is it that no one sees me steal something then immediately accuses me when I run into their vision? First of all, are there any mods that fix this? I could understand maybe with pickpocketing, or if I left and came back to an area later they might realize something has gone missing from a container or no longer on a desk which would at least give me time to pawn off the items... but when I steal something from upstairs and then run downstairs and someone sees me and immediately accuses me with mere seconds inbetween... just ridiculous.

When something gets stolen the people in the area begin to investigate. The will stop and ask to search anyone they find in the area.

Simple solution is to have your thief unlinked from the group and have some other character of yours intentionally get searched. They will find nothing on that character and immediately end their investigation.

Does this have any effect on their attitude or dialogue interactions if they search you and don't find anything?

No affect whatsoever. But they (somehow) can tell if ANY of that character's items are stolen, so keep a character with a "clean" inventory to be searched if you wanna be extra safe.

Like Qiox said, unlink your thief. First play through I didn't bother stealing but this time I am. Kind of a silly thing to do, but I just pick pocket as much as I can and waypoint elsewhere with the thief. Waypoint shortcut on mini map makes escape nearly immediate. Sometimes the NPC never investigated, but like Qiox said just have your innocent people ready for search. Also, pick pocket everything you want the first time because apparently you can't pick pocket the same person more than once, in the same session.

I've discovered the "radar thief" behaviour by accident. Sometimes when clicking on the screen for navigation or otherwise, I've inadvertantly clicked on some crummy red item I had no intention of picking up in the first place (most of the good stuff is sold by merchants or dropped by mobs, not laying on an open table).

As others have said, NPCs know they've been robbed and will challenge anybody in the vercinity and ask to search them. You should allow them to do this, once your thief has made good their escape of course.

A basic strat for successfull thievery is:

*Split your thief from party.
*Engage target in dialog with another character
*Switch to separated thief character leaving main party locked in dialog
*Sneak into position with thief
*Rob target blind
*Sneak thief away and hide
*Break dialog with target
*Wait for target to notice theft and ask to search you
*Allow search

You can usually approach the target to start the dialog from a specific angle that makes the sneak-in easier too.
