There's also some course language, mature themes including references to slavery and genocide, allusions to and characters that hold racist beliefs (Typically anti-lizard/dwarf/elf racial slurs), and a slew of other objectionable content not suited for younger or more squeamish viewers.
Just because it's not the worst/most graphic depiction of these acts and situations doesn't mean it's acceptable for children. I could say, a censored version could get a T, but that would potentially neuter some aspects of the story for several characters as the themes behind their personal stories get incredibly dark and brutal, including Ifan ben-Mezd.
It definitely takes someone who is ready for this kind of content to be able to really appreciate it, as well. If you're not the kind of person who is ready for or wants to experience the dark and grim themes in this game, you probably won't enjoy the game.
I did feel my heart swell with joy when I got the ending and epilogue for Happily Emmie After (A fan mod that lets you reunite two dogs separated by the Magisters in Fort Joy). The fact that it takes a mod to give two doggos a happy ending instead of a bitter one should make it clear that this game gets... Pretty grim.
Kids can handle plenty even more so in these times, and ya I can agree with to fully appreciate it if helps to be mature of course, but it as a whole really not much to it.

I'd say having a gameplay benefit for eating dismembered body parts is a hell of a hard M rating compared to a lot of other things.
It true that it fits M rating as I even said, but it not like it really extreme ie if this was adult anime content I wouldn't even call it a ecchi or for those unfamiliar with anime word "really softcore content".
But it doesn't depend on any of our takes on what Mature includes. That's what ESRB is for.
