*Spoilers* Cathedral or Vault first?

I've done the path of blood, and the puzzle under Cath. Ive also found Kemms vault but have not been back to clear it out yet. I have the painting from the trolls.

Which is best to do first? Kemms vault for Arhu or continue with Cath (currently @ line of anguish under lucians tomb)

I also havent been to see the doctor yet but I feel like i'm getting close to the end. So which order to tie everything off nicely?

Cath/Kemm/Doctor??
