The Doctor is unbeatably OP... (Spoilers...)
So I got to The Doctor... and I legitimately have no way to win...

I go and talk to him with Lohse, and I try to fight from within. Get rekt.
Reload
I try to fall back to the entrance, Lohse gets permanently charmed, I'm alone. Get rekt.
Reload
I try to fall back and keep Lohse at a distance and do guerilla warfare by falling back to fight.
Cannot outrun any enemies while using Cloak and Dagger, Tactical Retreat AND Flight.. Get rekt.
So...
Is this supposed to be unbeatable.... or...?

Fighting 6+ level 18s and 4 level 20s is hard enough, but the boss becomes a Demon, who then has 30k+ armor and 17k hp, and once he charms Lohse, he becomes Invulnerable...

Nothing I do seems to matter in the end, and I can't seem to get to anyone within town to try and recruit some backup... So I'm not entirely certain what else to do...

There are no chat options when talking to The Doctor... it's just '(continue)' until he attacks.

Absolutely terribly designed fight. Arrow storm doesn't actually touch him, neither does the storm spell. They do work against the minions though, so that's something.
Getting a game over screen because a heavily defended enemy doesn't die within two turns is pathetic. Only way I could do it was to kill Loshe to avoid the game over screen.
Horrible design.

In this screenshot, Beast's turn would be moving into the dissection room to prevent the nurse from taking the route around into the office and into the main hallway to reach Beast.

But basically what happened is that I don't have Lohse in my party, I didn't come across anything that indicated anything about candles, and I didn't get a context in which I could say his name to weaken him or whatever. For my party, he wanted to talk to Beast, who was a Scoundrel for my playthrough. The gist is after The Doctor starts the fight, the character he's talking to makes an escape (Adrenaline + Teleporter Pyramid, Escapist, whatever works). Later The Doctor/Adramalihk is going to stroll out of his office as a part of his patrol path.

If you start a fight with him out of his office you leave out the squad of Nemesis fighters entirely. So you fight with just Adramalihk and maybe a nurse as an extra. If you do get an extra nurse you have to make sure the route they pick isn't around and into the office.

With just that he doesn't get the damage bonus from Thick of the Fight. The fight became extremely straightfoward since mostly he spends his AP drinking resistance/buff potions.

My brother and I cheesed this fight big time. We stayed in the lobby and killed all the nurses, waiting for them to come to us and using raged attacks. Do not leave the lobby! Then he sent our rogue in and just did his most powerful move on the doctor, then cloak and dagger'd into the hall and ran toward the lobby until he left the fight. We rinsed and repeated this until the doctor had no armour left (he regenerates health out of battle). Then we all went in and stun locked him with knock downs. Was pretty easy but definitely earned the term 'cheesing'

Just beat him without snuffing any candles with Lohse as my main character on Classic at lvl 19.

I did manage to kill all of the nurses (who are named after the musical scales it seems?) beforehand. Then I did the voidwoken fish exchange thing (thank god I still kept that thing!), patted Jahar on the head (thanks a lot bub...) and used the side room that is adjacent to the dungeon hatch as a pull area.

I just had Lohse get whacked a few times to get her down to low health, then pulled. There's two full turns that pass before Adramahlihk uses the possession move, so just have to kill Lohse before that or Game Over. Then once Adramahlihk completes his attack round rez her and repeat. I was actually able to do enough physical damage to get through his armor and start my Knockdown chain before having to kill Lohse twice, so she was up and able to contribute for the rest of the fight. :)

I don't know if it still works or it got patched but Lohse can counter the charm spell by drinking potions of strong will (Immunity to Charmed).

the truth is, i killed him in his non-weakened state in 2 rounds. his minions are more annoying honestly :)

I killed him in 2 turns without having any issues. Do the candle quest before hand, if you have Lohse in your party Malady should appear in front of his house to trigger that.
I was level 20 on tactician; I had Ifan as ranger, Beast as 2H - necro and Lohse and Fane as mages. I casted meteor shower and thudnerstorm in round one and that was basically it. He survived with almost no HP, I spared him for Lohse so she could take revenge.
The fight is difficult on paper, because there are a lot of enemies and they have strong magic attacks. But you can just burst him down as with all enemies in this game.

Is this fight harder than the last fight? If yes, then should I use Anathema here?

So how do you guarantee going first? And I am pretty sure he's not killable with one volley of Arrow Storm - at least in Tactical difficulty.

There are a couple of ways to beat him honestly. Easiest is to kill the maids first then go for his physical armor. High fire resistance is a def plus on this fight. Really just sounds like you are undergeared/underleveled. Just do some stuff then come back to him. I know when I reached him I was level 19 with a party full of divinity gear.

You can always do the good ol' teleporting snowman.
1. Send one character to start a dialogue with the doctor.
2. Cast Cryotherapy on him
3. To trigger a fight, he needs to take damage. He is immune to damage while in cryotherapy, and the buff won't run out as long as he is in a dialogue. So you can teleport him as often as you like, to literally anywhere in Arx.
4. Bring him somewhere where where he is alone and has the low ground, e.g. the bridge into Arx.
5. He can't do anything the first round because of cryotherapy. Delay your turn(s) and then nuke him in his human form.

Some might consider this maximum cheese.

Ok, I killed him when I returned at level 20. I can see why people find this fight difficult; the Doctor has a crap ton of armor and health. So if you don't have enough burst damage and cannot kill him quickly, then I see how he and his adds can overwhelm you. Also positioning is important, as he didn't get to do anything his first (and only) turn, because he was too far from my group.

I unknowingly cheesed this fight my first run through by having a helmet equipped to Lohse that gave her immunity to being charmed. Charming Lohse is his only attack if she is in your party so I just sat there beating him up while he did 0 damage to my party.

I ran warrior/rogue/summoner/ranger on tact and melted his face. Granted I blew out the candles
