Report a translation problem
Getting a game over screen because a heavily defended enemy doesn't die within two turns is pathetic. Only way I could do it was to kill Loshe to avoid the game over screen.
Horrible design.

In this screenshot, Beast's turn would be moving into the dissection room to prevent the nurse from taking the route around into the office and into the main hallway to reach Beast.
But basically what happened is that I don't have Lohse in my party, I didn't come across anything that indicated anything about candles, and I didn't get a context in which I could say his name to weaken him or whatever. For my party, he wanted to talk to Beast, who was a Scoundrel for my playthrough. The gist is after The Doctor starts the fight, the character he's talking to makes an escape (Adrenaline + Teleporter Pyramid, Escapist, whatever works). Later The Doctor/Adramalihk is going to stroll out of his office as a part of his patrol path.
If you start a fight with him out of his office you leave out the squad of Nemesis fighters entirely. So you fight with just Adramalihk and maybe a nurse as an extra. If you do get an extra nurse you have to make sure the route they pick isn't around and into the office.
With just that he doesn't get the damage bonus from Thick of the Fight. The fight became extremely straightfoward since mostly he spends his AP drinking resistance/buff potions.
I did manage to kill all of the nurses (who are named after the musical scales it seems?) beforehand. Then I did the voidwoken fish exchange thing (thank god I still kept that thing!), patted Jahar on the head (thanks a lot bub...) and used the side room that is adjacent to the dungeon hatch as a pull area.
I just had Lohse get whacked a few times to get her down to low health, then pulled. There's two full turns that pass before Adramahlihk uses the possession move, so just have to kill Lohse before that or Game Over. Then once Adramahlihk completes his attack round rez her and repeat. I was actually able to do enough physical damage to get through his armor and start my Knockdown chain before having to kill Lohse twice, so she was up and able to contribute for the rest of the fight. :)

I was level 20 on tactician; I had Ifan as ranger, Beast as 2H - necro and Lohse and Fane as mages. I casted meteor shower and thudnerstorm in round one and that was basically it. He survived with almost no HP, I spared him for Lohse so she could take revenge.
The fight is difficult on paper, because there are a lot of enemies and they have strong magic attacks. But you can just burst him down as with all enemies in this game.
Is this fight harder than the last fight? If yes, then should I use Anathema here?
So how do you guarantee going first? And I am pretty sure he's not killable with one volley of Arrow Storm - at least in Tactical difficulty.
1. Send one character to start a dialogue with the doctor.
2. Cast Cryotherapy on him
3. To trigger a fight, he needs to take damage. He is immune to damage while in cryotherapy, and the buff won't run out as long as he is in a dialogue. So you can teleport him as often as you like, to literally anywhere in Arx.
4. Bring him somewhere where where he is alone and has the low ground, e.g. the bridge into Arx.
5. He can't do anything the first round because of cryotherapy. Delay your turn(s) and then nuke him in his human form.
Some might consider this maximum cheese.
