P.S. The green body shader effect not deactivating is indeed a BUG I have seen it happen as well.
Truly game-breaking bug.
One thing I will note is that I didn't have Fane while I was in the Fort. I swapped him with Ifan once I boarded the ship.
He was in human form when I used the mirror, and afterwards he was in his human form with a purple texture and a purple human portrait. I was able to transform him into any of the 4 transformations and that fixed his texture/portrait, but reverting to skeletal form has him as a purple skeleton with a purple human portrait. I can confirm that Vermillion's method works to fix these graphical errors.
The issue with not being able to talk to Fane while transformed is if not intended, at least a known bug. You can't talk to any of the characters while they are transformed, just fix it by reverting to their original form. Annoying, but not game breaking.
I'm not sure that the green transformation sheen is a bug, necessarily, but I have been able to fix it pretty reliably by just transforming again into his current form.
