Well, I do'nt have a map for you, but you know the part in the dry reef where you first see rock slimes? Theres a tunnel in that cliff face which leads to a door. If you unlock that door with a slime key, you can get into the Indigo Quarry.

If you walk out the ranch and turn right (not the first way, the second with all the blue rock slimes) there is a door inside a little cave. Behind that if you turn left over the broken bridge you'll find rad slimes. But be carefull with the radiation :)

you get it by

feeding a rad slime, just go through the cave when you find the first tabby slimes, and remember to take a slime key
