Download the latest drivers for both your Intel and Nvidia chip:

Once the downloads are done, uninstall both the Intel and the Nvidia drivers from your computer through. If asked, make sure to delete all settings.
Then restart your PC and install the two drivers which you have downloaded previously, start with the Intel and then install the Nvidia drivers. Then restart your PC.
Open up the Nvidia control panel and in the "Manage 3D settings" option try setting wallpaper32.exe to "High performance" mode like in this screenshot: https://i.stack.imgur.com/qeQAq.png Restart Wallpaper Engine if it was already running. If that does not work, try using the "Integrated graphics" option and see if that's better on your system.
Could you run the scan tool and share the log file one more time?
Can you open up task manager, sort by GPU usage and then take a screenshot that shows exactly what's using your GPU?
