Just do the Zaveid Sidequest and you will eventually able to access it. The sidequest unlocks near end game

Hexen Isle can be access without clearing the game.

Firsr, Clear Zaveid related quest, after that, to make sure, complete the quest by return to family in Taliesin, or directly go to Stonebury and talk to Exorcist there. Arter that, Hexen Isle can be accessed to finish another side quest and 1 code red daemon.
