Cant' find the last crest need help

I think i'm doing it wrong.I've got 3 crests b4 i entered the memorial zone and killed the witch which gave me the ability to tranform to a cat.So i think i might have missed a lot of stuffs that required cat form to get and the witch didn't give me the last crest so i'm a bit lost.I don't know where to look and have been backtracking for a while now.

You have the right idea - once you have the Cat Sphere there's a new area that you can get into back in the Sacred Ordalia Grove that you were too large to fit into before. You can find it near a save bell just a little ways outside of Karst City.
