Talk to all of the district residents in order to unlock both hints.s
No need to talk to all of them to unlock the option to let him die, just Carolyn Price (the woman in the shop near Aloysous' house) and Usher Talltree, the guy with the turban.
Rome wasnt built in a day, Trump's wall is still not built but Aloysius's wall merely takes a Following Night.
It doesnt sound like it makes any differentiation from the 'Wealthy" NPCs he aims to save from those that are "Poor" he wants isolate.
I didnt notice it happen in my game but that may of been because they were already sick.
And I've no interest in confirming the event either but it could just be another nonesense consequence that fails to matter other than to quirp "Oh noe. District Health!" and attempt to suddenly sink players that have Embraced a few NPCs.
Where do you get these hints ?
Or is that me ?
Usualy means you spam that chat option while running around to discover everybody anyway.
I'm asking because I'll be doing a pacificst run next :P
To be safe trigger everyone once the mission starts, the game forces you into talking to a few of NPCs anyway just track down the rest.
