You will know for sure if you messed up or not when you talk to the horned demon right before the final boss fight, as it will say something like 'you're too weak to face the challange ahead because you have refused to feed on anyone'. If you get that line you're golden.
And yeah, it could sound like the priest might be the reason.. Either that or because you hypnotized Crane. There are contradicting information about both of those cases affecting the outcome of the achievement, but sadly I can't talk from personal experience since I got it in first try and choose a different option =/
he said i was not prepared and i still got the achievement, haven't fed on anyone including the priest. forgot to save one dude from skals and had Crane turned into skal and killed.
turned Swansea and Aloysisus, let Sean drink my blood.
I got it with the following:
1. Priest spared during Mary fight
2. Made a deal with Dorothy. Didn't mesmerize her
3. Made vampires of: Swansea and Aloysius...they cost 5k exp
4. McCullum: didn't turn him
5. Sean spared
And ofc didn't embrace any of the named civilians.
Turned Sean Hampton.
Charmed Aloysius into accepting death.
Didn't murder the priest during the Mary fight (clearly that would be killing an innocent).
Spared McCullum.
Let Swansea die.
And of course, didn't kill any townsfolk.
From what I understand, as long as you don't outright kill any NPC's (including the unconcious priest) you should be fine for the achievement. LETTING someone die (like with Aloysius or Swansea) won't affect it, as you're not killing them yourself. Turning them doesn't seem to be the same as killing.
As for the actual ending, it seems to be turning people that makes Ashbury see you as a friend instead of a lover. Because from what I've seen, people who've done the same as I have but who turned either McCullum or Swansea get Ashbury feeling betrayed (because you're giving in and making more vampires).
I did the following:
1. I didn't embrace any citizen
2. I didn't kill priest in Mary fight
3. I spared Crane
4. I let Sean go and killed him and 3 others the next day when they went berserk
4. I made vampires from doctor and McCullum
5. I let Aloysius with his own death
I always treated every sick citizen and kept everything under control. No one died except for the mishap with the Docks and Sean because I didn't feed him my blood.
