about not even once achievement
I spare Crane, Sean(didn't turn him), McCullum and turn Swansea and Aloysius as every guide said, I didn't kill the priest during mary's fight and he was killed by mary. Since I didn't give Sean my blood and he ended up a beast, the district health level went down to criticle, Dyson Delaney and Paxton sisters went missing, is that an issue? Also Carolyn Price's daughter is unknown to me since she was kept in a locked room. I mesmerized several citizens to let them allow me into their house and I definitely didn't embrace anyone except for those went missing themselves. I ended up with level 26 and final boss of level 30 just as the pacifist run video guide I saw. How come I still didn't get the achievement?

Maybe because you lost the docks. and you turned aloysius,
you get the achievement for getting the best ending which require to not embrace anyone, but there might be additional requirement.

Interesting. I spared Crane, turned sean, spared McCullum, turned Aloysius although it's best if you have the hint that allows you to let him be happy to die but this play through I didn't find that one hint so I had no choice but to turn him, and I also turned Swansea and I let Mary kill the priest not me. I had a good ending and got the Not Even Once.

Once your at the point where everything is good don't sleep. Or make sure every NPC is treated with medicine then sleep. I think I only had 1 or 2 (NPC's) that died and it wasn't by me and sean was missing so I didn't sleep and all was good. If your docks went hostile thats not what I did and I got the achievement. It should be as simple as don't embrace any talking, Named NPC! Like at the beginning of the game when the game teaches you about the embrace with Cox. Don't do it, ever if you want that achievement.

you can turn them all it won't effect a pacifist run. as long as you don't embrace anyone on the citizen page and none die.

sparing sean turns him into a crazed skal same with charming crane. the missing people count against you because you will eventually have to kill them if you go to their area.

I didn’t kill those missing people, do I need to? I saw someone said before the final boss if your maker say sonething like ‘you’re too weak because you refuse to take a life’ then you’re guaranteed the achievement. Did you get that line?

Thank you for the help. I think not turning sean compromises the achievement since I’m sure I didn’t embrace anyone and all districts health level above hostile. But I think turning aloysius is a must, since that’s the only way for him to live.

I always treated every sick citizen and kept everything under control. No one died except for the mishap with the Docks and Sean because I didn't feed him my blood.

What about your districts health level? Did you mesmerize citizen so that they allow you into their house?

Hello,

The achievement will activate if you don't Embrace any Citizens throughout the game - you also need to not kill the Vicar during the boss fight with Mary.

Thank you. That’s clear enough condition so am I right to assume all other things shouldn’t effect the achievement, as long as I don’t kill citizen for exp and let Mary kill the vicar?

My districts are all sanitized but the one Aloysius died in, and his is healthy, and everyone healed when he died, best outcome ever..

It's very very clear - don't directly kill/embrace any non-aggro npcs. Other choices are up to you. Turning someone into vampire doesn't count. Done.
