It's deduced that Sean attacked and kidnapped Harriet since he was bitten and attacked by a Skal in the beginning of the game. Skals can create other skals through contact.
But it ended up being Mary stalking John the whole time and throwing hints and bodies at him until their final fight. But Ekons can create Skals if their blood isn't pure/strong enough.
You're not far enough
But if you like to know (but I'd really recommend not checking the spoiler!)
Harriet Jones was infected by a blood transfusion which was used by Dr. Swansea. A vampire gave this blood transfusion to him for research - but it was actually contaminated by a specific virus strain. Due to this contamination Harriet Jones actually transformed into a mutated version of a Skal - a much more troublesome version. She later infected her daughter, which became the source of infection for this mutation in the West End District.
