He's actually turning into a good guy again who doesn't fear death anymore. He is going to made peace with himself and uses his money to help the people of london before he dies soon after. This actually means that everyone in every district is healed.
But you'll be a traitor of Ascalon and there'll be some members of ascalon who'll try to kill you (just common vampires, nothing special). But this doesn't make much difference regarding the story later on.
I usually find out for myself in games, as I should, but I've already made terrible decisions I regret and hurt my enjoyment of the game (completely my fault though).
"They" (He, really) give(s) it to you regardless. Only difference is that you have a fight a high level Ekon (sp) to get to him to ask for it if you are an enemy of the group.
If you charm him into accepting his death, he donates all he has to healing people and the city gets instantly healed, and you get kicked out of the club.
If you turn him, most people instantly get sick, and you stay in the club, but Lady Ashbury now thinks you've betrayed her.
Betraying Ashbury cuts off the best ending.
Getting kicked out of the club doesn't matter. You'll need to go back to the club for something for a later quest, but if you're not a member you just have to fight a vamp to get it instead of walking in and having it handed to you (so honestly it's a positive as you get the drops from the vamp).
Turning Aloysius is better for "Pacifist run" and Not even once achievement:
+: If you turn Aloysius, it's easier for pacifist run because there will be less violence later. And since you have lower level, it's quite helpful not to fight more enemies.
+: Aloysius' district stays healthy.
- and +: Other districts become ill but since nobody is killed (except from bad major decisions which don't affect the achievement and ending), they won't become hostile. It might not look like that at first but this is really great because you'll get quite a lot of XPs from it.
+: Betraying Ashbury by saving Aloysius affects the ending (one or two sentences she says) but not the ending cutscene so you still get the best ending and Not even once achievement. Since nobody is killed, you can still persuade her and have the best ending. Even though Aloysius is evil, it should be in "pacifist" Reid's sake to save all pillars (which I didn't achieve because of the first 2 bad decisions which I regretted later very much but they gave me a huge lesson). If all districts are ill, it's reversible by healing them. If Aloysius dies, its district status is lowered by 20% and you can't do anything about it.
TL; DR: I charmed Dorothy Crane (huge regret), spared Sean Hampton (4 people died, saving Aloysius lowered Sean's distrcit to 50% but I could still save it from collapsing to hostile), turned Aloysius and turned Swansea. No human was killed, only citizens related to charmed Crane and spared Sean. I didn't turn McCullum either which is also helpful because when you meet him then, there won't be any enemies nearby.
Best ending, Not even once achievement, lvl 28, 56 hours. And I enjoyed it much more by making mistakes, regretting them and learning lessons from them. Great game.
If you want to know more about citizens, don't bother to find all hints during the "pacifist" playthrough. It's better to try to find all hints during "kill all" playthrough. You don't need hints during the "pacifist" one.
