It's been awhile and I'm not saying this is THE answer, I just remember the scenes seeming to focus on this blood enough for me to notice and wonder.
I'm guessing it has to do with either Frank getting stung during the intro to queens, and/or maybe the blood splatter from a zombie before fight Carlito with a sniper like other people here have mentioned. Or maybe Franks plot armour is not as strong as Chucks.
Is he completely cured in DR4? Haven't played it. But he did have to find Zombrex in Case West and Off the Record, so his infection played a role there.
In zombie games you can get bitten a hundred times and be ok but if it happens during a cutscene then you get infected.
It's like in an action game like Uncharted, you get shot all the time but are fine unless the story needs you to be actually hurt.
