I can't see a mod that turns off elevator maintenance, that would be nice to try and see if it confirms things.
Elevator breakdown is directly affected by the amount of traffic an elevator sees. You can clearly see when an elevator breaks down by the visual smoke. If you see a lot of breakdowns, you might need more elevators. Although you could still have times where all elevators will be shut down at one point or another (especially in a crowded tower). Place maintenance offices vertically throughout your tower to help them reach the floors easier. Don't keep them all in the basement. This could also be an issue of workers being stuck or not finding the path. But usually that results in an empty maintenance room, not one full of workers.
Be careful with that mod you could overdo it and place too many supply rooms and all your elevator use may be filled with workers. Haha. This is also true if you have a lot of service workers travelling. They can clog up the system so to speak. Again, spreading services vertically really helps with that.
