Quilin is so vulnerable

Basically just as vulnerable as any other pet. But because I don't ride pets I'm not used to taking care of them. Even if I did use them, I'd use them as extra hearts. I've lost him twice in 6-3 to the sparkling balls.

How do you usually handle this? Be extra careful? Clear the path in advance?

Be careful, try to clear a path in advance and verify that the route ahead is safe and remember that you can carry Qilin like any other mount (this will give you a slightly smaller hitbox compared to riding him). Progress through the level little by little and don't be afraid to drop him off at safe spots along the way to the exit.

Use bombs to destroy traps or create alternate paths that let you descend safely, after all, it's better to reach Sunken City with 0 bombs than not at all because you and/or Qilin is dead.

I also managed to lose him twice before actually reaching Tiamat. I'd say easiest solution would be taking your time, 3 mins are more than enough to just get down even when waiting for traps and etc. When you get to safe place always check what's below and don't spare on bombs to make shortcuts, even if you don't have much. It's better to get further in game with 0 bombs, than not getting there at all, am I right? Also I won't recommend leaving him behind and clearing path ahead before, since with all shenanigans going on everywhere in NB he might be already dead when you come to get him. Try always riding a turkey, they're good to avoid stupid damage in dwelling and will get you used to ride mounts.

Hopefully I get plenty of bombs if I'm planning a sunken city run, so I normally bomb a path and bring Qilin down that way.

I save all my bombs for Neo Babylon for this exact same reason. Also I usually grab the compass from the yeti king if i dont have one so i can just bomb my way to the exit so i dont have to deal with those anxiety inducing elevators and spark balls.
