Extra dog head?

Playing on MH, I spotted a key item with with the x-ray and went to find it was the white dog's head in its usual location. This is well AFTER I'd already made it to the yard. Has anyone else experienced this? Or is it something to do with MH?

More like a bug. I ran several all-items collect-athon runs on madhouse but never found a duplicate key item.

Weird. I might do another run to see if I can figure it out. My theory is that I solved the puzzle but didn't pick it up before I got the last (red?) one, but that's just an idea, not even a hunch lol. But if I can work out how I did it it might be notable to speedrunners. Oh, and thanks.

Lol, just had the same thing happen as well and brought me here. I just finished killing Jack and was in the Old House, decided to use the stims to run around the now safe Main House and thought maybe I found a secret lol. Womp womp. :(
