How Many Play Throughs Do I Need To Play?
in order to unlock everything in the game how many times do I need to play and what should I do for each play through?

Two. One on normal, one on the unlocked madhouse.
You need to collect all coins on both difficulties, destroy all bobbleheads (can be done in multiple playthroughs), and beat the game under 4 hours at least once (current speedrun records are around 1:30).

Unlocking everything in the DLC content takes longer. The two large story ones follow a similar path (one normal, one hard, be fast, find collectibles), but the minigames can require quite a few runs. I doubt anyone can unlock everything in 21 under 5-6 runs (especially since one of the unlocks is for beating a rare enemy twice, and it is random if he even appears at all).

Wouldnt it be 3? How can i get all the bobble heads all coins and beat in 4 hours with one normal walkthrough?

All Normal Coins while doing the 'less than 4 hours' run is possible, it most likely just won't happen on your first time unless you're playing with a guide. You can leave Mr. Everywheres for Madhouse.

How many playthroughs it requires will depend on you. The minimum possible is 2 (since it's impossible to get Normal Coins and Madhouse Coins in one playthrough), but that's only if you can combine multiple clear conditions in a single run, and of course that's if you have a game plan before even touching the game so no "Blind" first playthrough allowed. Otherwise it will take more.

Do it like I do it (I just finished Madhouse).

1: Enjoy the game on Normal, explore. If you desire, run with a guide to collect all collectables (Mr Everywhere, Coins & Documents, and watch all videos). Do the specific achievements in this run too, like the boss achievements for Jack (With the Chainsaw) and final Marguerite fight.

2: Clear the game on Madhouse, and collect all Coins again (More of them, and different locations).

3: Clear the game on Easy (While you've unlocked Infinite Ammo) and do it under 4 hours and without opening the storage crate. It's what I'm going to do next.

DLC takes a wile though. I'm almost done, DLC is surprisingly enjoyable, even the mini games.
